Every Moment Matters

At Primecare Hospice, we offer four levels of care.

Routine Home Care

Primecare is committed to helping patients spend as much time at home as possible. Our interdisciplinary team provides Routine Home Care services to patients in their residences. This could be their own home, your home, an assisted living facility, a nursing home, or another location. By administering care at home, patients can remain in a familiar, comfortable, and safe environment with minimal disruptions.

Continuous Care

Continuous Care is a type of care that takes place in the patient's home and is typically administered during a medical crisis. While routine home care focuses on managing daily needs and symptoms, Continuous Care is designed to address urgent needs that require immediate attention.


This level of care is defined by Medicare's guidelines, which state that it may only be provided during a crisis period when a patient requires continuous nursing care to relieve symptoms or manage acute medical conditions. Some of the symptoms that may require Continuous Care include:

  • severe pain,

  • bleeding,

  • acute respiratory distress,

  • nausea and vomiting, and

  • terminal agitation or restlessness.


Under Continuous Care, we provide a minimum of 8 hours of care during each 24 hours, and the service is revisited every day. It's important to note that Continuous Care is a short-term service, unlike Routine Care, and is one of two levels of care that fall under the category of "Supportive Care." The other level is Transitional Care.

Respite Care

Respite Care refers to a service in which the patient is temporarily placed in a contracted nursing facility, hospital, or inpatient facility, for up to five consecutive days. This type of care is designed to provide a break for the caregiver, allowing them to rest and attend to other obligations.


In the event of a medical emergency or if the caregiver needs to travel out of town, Respite Care offers a solution. Additionally, if the caregiver is experiencing burnout or compassion fatigue, Respite Care can provide a much-needed break for both the caretaker and the dependent, preventing serious health consequences.

In-Patient Care

There are instances when patients need to be treated in a clinical setting, such as a contracted hospital, nursing facility, or in-patient facility. Typically, in-patient care lasts no longer than eight days, and our objective is to alleviate symptoms so that patients can return to at-home care.


Here are some symptoms that may require in-patient care:

  • Severe acute pain that cannot be managed outside clinical settings

  • Nausea and vomiting that cannot be controlled outside clinical settings

  • Respiratory distress that requires clinical intervention

  • Continuous or complex wound care

  • Uncontrolled seizures

These are only a few examples; there are other cases that may necessitate in-patient care.
